Job Description:
Position Summary:
* In a collaborative and mulitdisciplinary environment, contributes
professional and clinical expertise to assigned patient populations. * Performs
risk stratification for cardiovascular disease on the adult population (21-85
years old) utilizing the ASCVD risk score. * Identifies age appropriate risk
assessment for cancer diagnoses. * Prescribes diagnostic testing and referrals
for further evaluation when clinical findings warrant.* Performs individual
counseling after assessment of identified risk This includes counseling on
identifies risk factors. This includes counseling on specific lifestyle changes
including heart health nutrition, weight loss, exercise, stress management, and
if indicated, optimizing blood pressure and diabetes management. * Identifies
patients who require additional nutritional counseling services and refers for
consultation.: * Identifies patients who require referral to appropriate
physician specialist (cardiologist, Heart Care for Women, primary care, OB, GI,
Cancer Center, Sleep Center etc.) for evaluation and management.: * Identifies
patients that require referral to the ER and coordinates initial evaluation.: *
Participates in community outreach activities including lectures to community
and corporate groups. Contributes to the growth of the program * Participates
and lectures at patient support groups (Women and Men’s Heart Support Groups).:
* Provides education and resources on cardiovascular risk factors and risk
factors for developing heart disease at health fairs. * Reviews patient satisfaction
data with director on a monthly basis.: * Along with director, ensures that
education and practices are evidence based.: * Maintains seat at Tumor Board.

Qualifications & Education

Reports To: Director Cardiac Surgery and Cardiac Specialty ProgramsQualifications: * 1. Current Registered Professional Nurse and Certification as an Advanced Practice Nurse in New Jersey. 2. Master’s Degree in Nursing required, Doctorate (DNP, PhD) preferred. 3. Minimum of 3-5 years of clinical nursing practice experience, primarily in area of specialty.
